Once you have obtained your state form(s), please follow the steps below to fill and upload them to this portal:

Step 1: Vendor Information:

Where the form prompts for details about the "Seller", "Vendor", or "Supplier", enter our information exactly as shown highlighted below:

Step 2: Define Certificate Type:

Mark the document as a "Blanket Certificate" if applicable. This allows us to keep your resale certificate on file for multiple orders. 

(NOTE: Certificates marked as "Single Use" or "Single Purchase"—  will be rejected).

Step 3: Description of Items Purchased

If the form asks you to define or describe the products you are purchasing, please write in: 

"Trading Cards & Accessories"

Step 4: Sign & Date:

Complete the remaining applicable fields, sign, date, and prepare for upload on the next page.
